Dr. Kainkaryam πŸ‘¨β€βš•οΈ is a designated US Civil Surgeon by USCIS (https://my.uscis.gov/findadoctor) and offers Immigration Physicals in our South Windsor and Rocky Hill locations.

Our immigration physicals are a two-part process, where the first part is to identify the various needs and complete the exam, and the second part is to follow-up on any tests/needs and complete the paperwork.

Typically, we can get you in to your immigration exams in 1-2 days, and the process start to finish is usually is completed in 1 week barring any abnormal lab tests.

Appointment Registration Steps

2. Register for the option that you would like using the following links on the right. Here are your options:

Adult Immigration Exam: $225

β€’ Labs can be billed to insurance or use discounted prepaid cash-pay options noted below
β€’ Option to include required labs in package: tuberculosis blood tests, syphilis screen, and gonorrhea screen for additional $125
β€’ Option to include MMR lab test in package: $50
β€’ Option to include Varicella (chicken pox) lab test in package: $50
β€’ Vaccines billed to insurance or cash pay option

Child Immigration Exam: $200

β€’ Labs can be billed to insurance or use discounted prepaid cash-pay option noted below

4. Complete, print and bring the pre-filled I-693 form. We will periodically check to make sure it is the most up to date.

First Visit

(1 hour)
Please bring the following:

Government-issued ID with picture (passport preferred)

Medical Records translated into English by certified translator. If you have had treatment for Tuberculosis or any other communicable disease, this information should be there.

Immunization/Vaccination Records translated into English by a certified translator

Mental Health Records translated into English by a certified translator

Interpreter – if you need interpreter services, you are required to bring a certified interpreter with you

I-693 Forms – DO NOT sign Section 2.

We will review your medical history, complete your exam, and order any necessary testing based on review of documents.

Second Visit

(20-30 minutes)
We will complete the paperwork and certify form I-693 and the intake form and provide the forms in a sealed envelope. DO NOT OPEN THE SEALED ENVELOPE – this must be turned in with your immigration application to the Department of Homeland Security. A copy of your I-693 report will be provided for you to keep after we have discussed the results. The exam is valid up to a year after the immigration exam date.

Additional Blood Tests
If, based on review of your documents, you need additional bloodwork, and you have insurance, you have two options. You must choose which option you want prior to ordering the tests.
1. Use your insurance to cover the costs of the test (which you may not know until you receive the bill from the lab).
2. Our practice will cover the costs of the tests and you would pay us the fixed rates listed below.

X-Rays
If you need an x-ray, you have two options. You must choose which option you want prior to ordering the tests.
1. Use your insurance to cover the costs of the test (which you may not know until you receive the bill) and go to an x-ray site of your choice.
2. Go to our imaging partner and pay them the cost listed below.

If, based on review of your documents, you need additional vaccinations, you may choose to get them at our practice (if we have it in stock at our costs below), or, if you are above the age of 18, you can get them at any Pharmacy, and the Pharmacy will bill you or your insurance. Since we do not accept or bill insurance, our prices are listed below. For children, some of the vaccines may be covered free through the Connecticut Vaccination Program depending on the age.
